    Additionally, the individual must have a documented disability or disabling condition as defined by the following criteria:

    • Aged, Blind, or Disabled: The individual is recognized as aged, blind, or having a disability under Title II of the Social Security Act.
    • Extended or Long-term Incapacitation: The individual has an injury or illness that is expected to cause extended or long-term incapacitation.
    • Developmental Disability or Mental Illness: The individual has a developmental disability, a related condition, or a mental illness.
    • Transition from Residential Care: The individual has a mental health condition, substance use disorder, or physical injury that previously required a residential level of care and is now transitioning to the community.
    • Learning Disability: The individual is determined to have a learning disability according to the policy adopted by the Department of Human Services (DHS).
